A patient care coordinator is responsible for meeting with patients and their carers/family members to discuss their requirements, explain the services that are available to them, and create a personalised patient care plan that is tailored to their specific needs.

As a patient care coordinator, you will include the patient and their careers or family in discussions about their care plan. Once the specific care plan has been created, you will monitor their progress and amend the plan as needed. It will also be your priority to ensure the provision of care provided is in line with best practice guidelines and procedures determined by the NHS Trust or healthcare organisation you are employed by.

As a patient care coordinator, you will liaise with other healthcare professionals to gain their input on the healthcare plan of your patients including physiotherapists, general practitioners, and osteopaths for example. You will be required to visit the patient periodically and ensure excellent lines of communication are maintained and keep accurate and concise records and documentation following every visit or communication. This is important so other healthcare professionals are aware of the progress the patient is making, any issues they are experiencing, and the medication administered.

Whilst caring for your patients as a care coordinator, you will demonstrate empathy, encourage awareness of their surroundings, develop and maintain an effective care plan, and work to improve their confidence levels whilst educating them so they can make the right decisions about their own health and well-being.
